What Dr. Rhonda Patrick Says About the Norwegian 4x4

Dr. Rhonda Patrick has repeatedly named the Norwegian 4x4 — four 4-minute intervals at 85–95% of max heart rate with 3-minute recoveries — as one of the most effective ways to raise VO2 max, which she considers among the best measurable indicators of how long and how well you will live.
She is not selling a program. She is a biomedical scientist (PhD, biomedical science) who runs FoundMyFitness and spends an unusual amount of time reading the primary literature. That is exactly why her take on the 4x4 is worth documenting carefully — with links, not vibes.
What did Rhonda Patrick actually say about the Norwegian 4x4?
The clearest statement is her own. In a December 8, 2023 post on X, she wrote that cardiorespiratory fitness, measured by VO2 max, is "one of the best indicators of improved healthspan and lifespan," and that the Norwegian 4x4 is "One of the best protocols for improving VO2 max." She described it the same way the studies do: a 4-minute interval at the highest intensity you can hold for the entire interval, a recovery period, repeated four times.
That is the same protocol tested in the Helgerud 2007 trial, where the 4x4 group improved VO2 max by 7.2% in 8 weeks and beat both lactate-threshold and long-slow-distance training. If the wording of her post sounds familiar, it is because she is describing the study protocol, not inventing her own.
How does she compare the 4x4 to other interval protocols?
In a FoundMyFitness episode with exercise physiology writer Brady Holmer — "These 3 Workouts Are Guaranteed to Increase Your VO2 Max" — Patrick and Holmer walk through three protocols side by side: the Norwegian 4x4, a 1-minute-on/1-minute-off session, and Tabata. Holmer's summary position in that discussion is that intervals in the 2–4 minute range are particularly effective for improving VO2 max, because they force you to spend meaningful time near maximal oxygen uptake. They also suggest rotating protocols across the week so you stress different energy systems rather than repeating one stimulus until it stops working.

Why does she treat VO2 max as a longevity metric?
Two reasons show up across her appearances on Diary of a CEO and Modern Wisdom with Chris Williamson.
First, the mortality data. In the Mandsager 2018 study of 122,007 patients, people with low cardiorespiratory fitness had roughly 4–5x the mortality risk of the most fit group, and the benefit kept climbing with fitness — no upper limit was found. Patrick cites this class of evidence constantly when she argues VO2 max deserves the same attention as blood pressure or cholesterol. Peter Attia makes the same argument from the same dataset.
Second, the reversal data. Patrick frequently points to the 2-year Dallas trial led by Dr. Benjamin Levine (Howden et al., Circulation 2018), in which 61 sedentary middle-aged adults trained progressively for two years, with a weekly 4x4 session as the intensity anchor inside 5–6 hours per week of mixed training. VO2 max rose about 18% and left-ventricular compliance — the stiffness of the heart muscle — improved by more than 25%. Her framing, in her own words from that coverage: the 50-year-olds "reversed their cardiac structure aging by 20 years" (via Fitness Volt). On Diary of a CEO she went as far as calling the 4x4 the "gold standard" for cardiorespiratory fitness, per Marie Claire's coverage.
One honest caveat she is careful about, and so am I: the 20-years-younger result came from the whole two-year program, not from the 4x4 alone. The 4x4 was one session per week inside a larger training load. More on what that study did and did not show in the VO2 max and longevity breakdown.
Does she say beginners should jump straight into the 4x4?
No — the opposite, and this caution is one of the most useful things she has said about the protocol. Discussing the Levine study, she pointed out that the sedentary participants did not start with the full 4x4 at all: "It took them six months to work up" (via Fitness Volt). An untrained person starting at four all-out 4-minute intervals is a recipe for quitting, or worse.
Her practical guidance across these appearances amounts to a progression:
- Build an aerobic base first with easier continuous cardio.
- Start with one or two hard intervals, not four, and shorter than 4 minutes if needed.
- Add intervals and length over weeks and months as recovery improves.
- Only then run the full protocol — and even then, 1–3 sessions a week. Once weekly was the studied dose in the 2-year trial.
That matches the trial designs. Progression was built into both Helgerud's and Levine's protocols; it is not an optional nicety.
How often does she say to do the 4x4?
Not daily. Across her posts and episodes the working range is one to three sessions per week, and the strongest evidence sits at the low end: once weekly was the studied dose in the 2-year Levine trial, surrounded by easier aerobic sessions on other days. The 4x4 is a high-stress stimulus. The adaptation happens in the recovery between sessions, and stacking hard interval days back to back mostly buys you fatigue, not fitness. Patrick's own training talk reflects that split — most weekly cardio volume easy, a small vigorous slice doing the VO2 max work.

How can you tell the protocol is working?
Patrick's answer is the same as the researchers': the interval quality and your heart's response, not the burn. You should reach 85–95% of max heart rate by the back half of each interval, and over months your heart rate at a given pace should fall while your recovery between intervals speeds up.
